Abstract
Tyrimo tikslas: įvertinti LSMUL KK Chirurgijos klinikoje 2015–2019 metais dėl pirminio hiperparatiroidizmo operuotų pacientų gydymo rezultatus.Tyrimo uždaviniai: 1. Apibūdinti pacientų operuotų dėl pirminio hiperparatiroidizmo kontigento charakteristiką. 2. Nustatyti tiriamųjų asmenų vyraujančius klinikinius, laboratorinius ir instrumentinius tyrimų duomenis. 3. Išanalizuoti pooperacinius Ca, PTH pokyčius bei įvertinti gydymo rezultatus. Metodai: LSMUL KK Chirurgijos klinikoje atliktas retrospektyvinis tyrimas. Jo metu buvo nagrinėjami 2015–2019 metais operuotų pacientų dėl pirminio hiperparatiroidizmo ligos istorijų duomenys. Statistinei duomenų analizei naudota SPSS 23.0 ir MS Excel programos. Pagal Kolmogorov – Smirnov kriterijų visi skirstiniai nuo normaliojo nesiskyrė, dėl to vidurkių lyginimui buvo naudotas Student t testas. Reikšmingumo lygmuo p < 0,05. Tyrimo dalyviai: pacientai, kurie 2015–2019 metais buvo operuoti dėl pirminio hiperparatiroidizmo. Tyrimo rezultatai: tiriamųjų imtyje buvo 97 (90 %) moterys ir 11 (10 %) vyrų. Vidutinis pacientų amžius – 60,99 (12,68) metai. Iš 108 tiriamųjų 17 (15,7 %) neturėjo jokių simptomų, o 91 (84,3 %) turėjo klinikinių simptomų. Ligos simptomų trukmės vidurkis – 21,27 (29,07) mėn. Tarp dažniausių klinikinių simptomų buvo osteopenija/osteoporozė 47 (43,5 %) tiriamiesiems, bendras silpnumas 38 (35,2 %), inkstų akmenligė 34 (31,5 %). Tarp tirtų pacientų dažniausia 77 (71,3 %) gretutinė liga buvo arterinė hipertenzija. Priešoperacinis PTH buvo visiems tiriamiesiems padidėjęs virš normos, jo vidurkis buvo nustatytas 33,75 (40,71) pmol/l. Priešoperacinio serumo Ca ir Ca2+ vidurkiai buvo nustatyti tokie: 2,85 (0,26) mmol/l ir 1,31 (0,136) mmol/l. Priešoperacinis P kiekis tiriamųjų kraujo serume normoje buvo 42 (38,9 %), o mažiau už normą 35 (32,4 %). Ultragarsinio tyrimo metu nustatytos padidintos prieskydinės liaukos ilgio vidurkis – 1,78 (1,01) cm, o skersmens – 1,13 (0,67) cm. Didžiajai daliai 91 (85,85 %) tiriamųjų UG buvo nustatyta vienos prieskydinės liaukos patologija, o dažniausia PLA lokalizacija buvo nustatyta dešinėje apačioje 37 (34,3 %) bei kairėje apačioje 35 (32,4 %). Buvo nustatytas UG ir scintigrafijos tyrimo jautrumas, atitinkamai 95,4 % ir 83,11 %. Šių tyrimų teigiama prognostinė vertė atitinkamai 81,4 % ir 90,14 %. UG ir scintigrafijos duomenys sutapo 66 (61,1 %) atvejų ir taip pat scintigrafijos tyrimas parodė, jog dažniausia PLA lokalizacija yra kairėje. Operacijų trukmė buvo vidutiniškai – 53 (18,1) min. Didžiajai daliai 97 (89,8 %) pacientų buvo pašalinta viena liauka, esanti apatinėje lokalizacijoje. Buvo nustatyti 31 (28,7 %) pacientai, kuriems buvo atlikta skydliaukės operacija kartu su paratiroidektomija. Pašalintų prieskydinių liaukų ilgio vidurkis 2,23 (1) cm, skersmuo 1,45 (0,65) cm, o svoris 2,17 (1,4) gramų. Po operacijos atliktas patohistologinis tyrimas visiems pacientams parodė, jog daugumai 89 (82,4 %) PHPT sukėlė prieskydinės liaukos adenoma. Po paratiroidektomijos 15–20 min PTH sumažėjo iki 4,88 (6,6) pmol/l, o praėjus 1 parai PTH sumažėjo iki 3,516 (12,7) pmol/l, o Ca sumažėjo iki 2,34 (0,19) mmol/l (p=0,0001). Pooperacinio Ca2+ vidurkis buvo nustatytas mažesnis už normą – 1,1 (0,1) mmol/l, o P vidurkis buvo atitinkantis normą – 0,96 (0,24) mmol/l. Daugumai 104 (96,3 %) pacientų nebuvo chirurginių komplikacijų, trims (2,78 %) buvo nustatytas vienspusis balso klosčių paralyžius: dviems iš jų kartu šalinant vieną PLA buvo atlikta ir hemitiroidektomija, o kitam dviejų skirtingų lokalizacijų PLA šalinimas. Išvados: 1. Pirminiu HPT dažniau sirgo pomenopauzinio amžiaus moterys. 2. Dauguma pacientų turėjo klasikinę simptominę PHPT formą, sąlygotą hiperkalcemijos ir PTH pertekliaus. Ultragarsinis tyrimas ir operacijos rezultatai parodė, jog daugumai pacientų PHPT sukelia viena apatinės lokalizacijos prieskydinės liaukos adenoma. 3. LSMU KK Chirurgijos klinikoje po atliktos paratiroidektomijos sėkmingai susinormalizavo serumo Ca ir PTH rodikliai, o pooperacinių komplikacijų dažnis neviršijo nurodyto literatūroje.
Aim of the study: to evaluate the results of the surgical treatment of primary hyperparathyroidism at the Clinic of Surgery of LUHS KC during the period 2015–2019. Research objectives: 1. To describe the contingent characteristics of patients operated for primary hyperparathyroidism. 2. To define the predominant clinical, laboratory and instrumental data of the patients. 3. To analyse postoperative changes in Ca, PTH levels and to evaluate the results of the treatment. Methods: a retrospective study was conducted at the of Surgery Clinic of LUHS KC. It analysed the medical records of patients operated for primary hyperparathyroidism during 2015–2019 period. SPSS 23.0 and MS Excel softwares were used for statistical data analysis. According to the Kolmogorov-Smirnov criterion, all probability distributions were not different from normal distribution therefore, the Student t test was used to compare means. Significance level p < 0.05. Participants of the research: patients who were operated for primary hyperparathyroidism during the period 2015–2019.
Research results: the sample consisted of 97 (90%) women and 11 (10%) men. The mean age of the patients was 60.99 (12,68) years. Of the 108 subjects, 17 (15,7%) had no symptoms and 91 (84,3%) had clinical symptoms. The mean duration of symptoms was 21,27 (29,07) months. The most common clinical symptoms were osteopenia/osteoporosis in 47 (43,5%) subjects, general weakness in 38 (35,2%), and kidney stones in 34 (31,5%). The most common comorbidity was arterial hypertension observed in 77 (71,3 %) of the patients. Preoperative PTH was increased above normal range in all subjects, with a mean value of 33,75 (40,71) pmol/l. Preoperative serum Ca and Ca2+ means were determined: 2.85 (0,26) mmol/l and 1.31 (0,136) mmol/l. The preoperative P level in serum within the normal range was in 42 (38,9%) subjects and below the normal range in 35 (32,4%). Ultrasound findings of the enlarged parathyroid glands showed the mean length of 1,78 (1,01) cm and a mean diameter of 1.13 (0,67) cm. The majority 91 (85,85%) of the subjects had a single abnormal parathyroid gland, diagnosed by US, with the most frequent parathyroid adenoma (PA) localization in the right lower position in 37 (34,3%) and in the left lower position in 35 (32,4%). The sensitivity of US and scintigraphy was 95,4% and 83,11%, respectively, and their positive predictive value was 81,4% and 90.14%. Ultrasound and scintigraphy data matched in 66 (61,1%) cases and scintigraphy also showed that the most frequent localization of PA was on the left. The average surgery time was 53 (18,1) minutes. The majority of the patiens 97 (89,8%) underwent removal of one gland of the inferior localization. There were 31 (28,7%) patients identified who underwent simultaneously thyroid surgery combined with parathyroidectomy. The mean length of the removed parathyroid glands was 2,23 (1) cm, diameter 1,45 (0,65) cm and weight 2,17 (1,4) g. Pathohistological analysis was performed for all patients and showed that majority (82.4%) of PHPT cases was caused by parathyroid adenoma. Immediately (15–20 min) after removal of the pathological glands PTH decreased to 4,88 (6,6) pmol/l and one day after surgery PTH decreased to 3,516 (12,7) pmol/l and Ca decreased to 2,34 (0,19) mmol/l (p=0.0001). The mean postoperative Ca2+ was found to be below normal range – 1.1 (0.1) mmol/l and the mean P level was normal – 0,96 (0,24) mmol/l. The majority of the patients 104 (96,3%) had no surgical complications, three (2,78%) had unilateral vocal cord paralysis: two of them underwent removal of one PLA while performing hemithyroidectomy, and the other underwent removal of two PA from different localisations. Conclusions: 1. Primary HPT was more common in postmenopausal age women. 2. Most patients had the classic symptomatic form of PHPT due to hypercalcaemia and PTH excess. Ultrasonography and surgical findings showed that the majority of patients had PHPT caused by a single inferiorly located parathyroid adenoma. 3. After parathyroidectomy performed in Surgery clinic of LUHS KC, serum Ca and PTH levels were successfully normalised and the incidence of postoperative complications did not exceed those reported in the literature.
